Mental health concerns impact on a person’s ability to manage every day life.
The Reconnect Mental Health Service is an outreach, early intervention service, supporting young people aged 12 to 18 years and their families, where there is a risk of homelessness and emerging mental health concerns for the young person.
We work with them to identify emerging issues early in the piece and work therapeutically with them – and their families – for up to six months. We provide support in terms of counseling, family work, alcohol or other drug counseling and make introductions with other community services as needed.
Reconnect is a voluntary program which offers confidential support.
Who: Young people aged 12 to 18 years where there is a risk of homeless and emerging mental health concerns.
Where: This mobile service offers support to young people across metropolitan Adelaide in their home, school or another familiar location.
How: Referrals are accepted from individuals, parents, teachers, school counsellors, service providers and other support people.
