National Psychosocial Transition Supports Program (NPTS) provides psychosocial support services to people affected by mental illness who are not currently supported through the NDIS.
The program is for people who were being supported through the Personal Helpers and Mentors Program (PHaMs).
NPTS supports people whose life is significantly affected by mental illness. The focus is on the individual’s recovery by providing them with opportunities, support and services to help them reconnect to community.
Centacare delivers the service in Salisbury, Tea Tree Gully and Murray Bridge.
We facilitate mental health recovery and self-determination through individualised holistic and strength-based support.

We recognise that recovery is a journey which embraces peer support, hope and community engagement.
